my former employer (a popular online bookstore) had an internal memo system called "COE", for "correction of error", where teams would explain in excruciating detail how they had accidentally taken down important services for minutes or hours. There was at least one time while I was there where the website was inaccessible for hours because someone had manually deleted a virtual network resource because someone asked them to, in an attempt to "clean something up"
