I thought it was all very straightforward, honestly (also SPOILERS) (lol):
- There is one level of fantasy that plays itself out where, in the instant before she gets Hammed, Baby Doll mentally replays the events of the past five days cast in the form of the burlesque prison.- When Baby Doll dances in the burlesque fantasy, she drops into the action sequence fantasies which represent her crew's attempts to assemble all of the pieces necessary for their escape.
In actual reality, at least three of the girls were seriously hurt and/or killed during the breakout attempt, the aftermath of which can be seen in the sweep through the sanitarium after Baby Doll gets Hammed. Also, you see that a good chunk of the interaction between the orderly and Dr Carla Guguino never actually happened and the orderlies were terrorizing the girls behind the doctor's back and without her knowledge. We also know from actual reality that:
- Rocket died in a struggle with the cook.- Baby Doll helped Sweet Pea escape and gave herself up as a distraction.- The main awful orderly tried to force himself on Baby Doll and she stabbed him in the neck.
Everything else, including possibly the existence of Amber and Blondie was warped through how Baby Doll experienced life in the sanitarium; in reality, there was no collusion whatsoever between the doctor and the orderlies, the orderlies had no power over the doctor, and Jon Hamm was just a handsome, pensive dude handing out a lobotomy or two.
The point where there's "... or DID THEY????" confusion is the casting of the bus driver as the girls' mentor in Baby Doll's fantasy, which you can either read as the script signposting that none of this ever actually happened, including the second half of "reality", and Baby Doll is just in a blissed-out lobotomized haze with no free friends and no impending justice against her stepfather, or as the script being lazy in trying to be clever clever about aping its Wizard of Oz double-casting.

im somewhat serious about that - i do think the movie was meant to indict male gaze-iness in nerd culture by inviting dudes to watch hot babes and guns and explosions and all that crap, but then pulling a switcheroo on em with all the feelbad stuff. its just unimaginably stupid and tasteless in the execution

I thought I would really like this movie but when I finally saw it, the battle scenes were uninteresting to me, just manufactured CGI crap that went on for 3x too long. So sad.
