Free Library of Philadelphia has it going back at least ten years, probably a little further. I don't know about indexing. Did you check Ulrich's? That will tell you where things are indexed.

I would think lots of libraries in the US would have a complete run though.

erm actually i have a complete run - but i wd be a bit surprised if many US libraries have all of the first 20 or so issues, it was VERY er "underground" to start with

there is a somewhat cranky online index here, which begins w.#101: there is (or was) an actual real published index for the first 100 issues tho whether it is still available i couldn't say (it is pre-digital)

By "lots" I meant "maybe a couple." Seriously, though, there must be some who have it on microfilm. (Is it commercially available on microfilm? I could check all of these things in about 20 minutes.)
