Sanchez was drilled after hitting a bunch of home runs this series, then Kahnle was ejected for throwing near Cabrera (which understandably made Girardi irate enough to be ejected), then Cabrera appeared to go after Romine for just being there. All bad.

The above clips don't show Cabrera talking at and stepping toward Romine (not the plate) just beforehand.

And it's possible this bullshit was carried over from HBP exchanges of the previous series.

Would like to think Fulmer's smart enough to not throw intentionally. He did act like it got away from him -- enough to prompt a trainer visit -- for whatever that's worth.
