It's not really any different from telling a 'fat joke' and having a friend with obesity walk by and be hurt and you saying "oh I wasn't talking about YOU".

the 'being fat' is the joke and is, as such, punching down. making fun of Trump for lying about his weight seems to be more mocking him for lying about everything, even unimportant things, than the actual being fat. possibly borderline but at least I understand that better than 'hur hur when Trump sits around the house..." jokes.
