The standard for overturning the referee's original decision is that there has been a "clear error", sometimes expanded to "clear and obvious error"

don't disagree that it can work, but every game I've seen it in so far, the refs seem terrified not to use it

happy to revisit it when it's run by some infallible AI that can make decisions instantaneously
