Supposedly this album, "Paranoia Picknick" by Ernst Schultz, fits the bill, though as all the lyrics are in German I couldn't say for sure:

It's a huge photo and a bit NSFW, so I'll only link to it, but the booklet for Ghost's Infestissumam seems to fit the bill.

Woody's newspaper says "Sunday" on the front, so I guess he's meant to be Sunday Morning (also the lyric about "glass of fizzy water helps to start the day")?

The Temptations - Greatest Hits II. Kind of a sad example of the form - feels like SOMEONE had faith in the big letters to convey the idea, but someone else insisted on the band/title being repeated at the top with a group photo so small it can't satisfy any fan, in the process shortchanging the main concept.

Billy Joel - Turnstiles (1976). According to Joel, each of the characters on the album cover was meant to represent a particular song (e.g., the girl in headphones for "All You Wanna Do is Dance," the wealthy couple for "I've Loved These Days").[2]

Case by case basis imho - I feel like with a lot of those the images are so abstract or vibe-oriented that they don't quite count as "illustrations" if you catch my meaning. Some pinch of literalism (bordering on the ridiculous) is essential.

At least some of of the ones in Superunknown clearly count for example - "Let Me Drown" shows a pool, "Head Down" is a photo of a fetus with its head towards the bottom of the page, etc.

I know it isn't precisely what is being asked for, but I always liked all the different photos/images in the Superunknown liner notes for each song's lyrics.

The album's outside cover consists of the words 'King Crimson' spelled out in ornate medieval lettering, the word 'King' on the back cover and the word 'Crimson' on the front cover, with each letter incorporating one or two discrete images. These images in turn represent Sinfield's lyrics from the album – the images in the word 'King' representing the lyrics of the various sections and subsections of track 5, "Lizard"; while the images in the word 'Crimson' represent the lyrics of tracks 1–4. Whereas the images representing "Lizard" are medieval in content – depicting Prince Rupert, his environs (including a peacock), and the Battle of Glass Tears – the images representing the other four tracks juxtapose medieval and contemporary scenes. The image around the letter 'i' in 'Crimson', for example, depicts the Beatles, corresponding with their pseudonymous appearance in the lyrics to "Happy Family". Around the "n" on the front cover, there is a depiction of Rupert the Bear piloting a yellow aeroplane.

I'm not sure if all the songs are illustrated here, but all the illustrations are certainly taken from the lyrics on the album... the satyr from the title track, the old woman in the mirror from Ripples, the huntsman from Squonk, etc.

ELP's Tarkus has this in the gatefold, seven drawings which correspond to the different sections of the title track, although the lyrics don't really match the drawings. though they do match the titles of the section. it's kinda weird, I suspect there's no overarching concept just the band doing two things at once
