I know that Allred provide an acknowledgments list on the title page, but a page-by-page and maybe panel-by-panel TOC would be nice, especially since I'm not familiar with most of the artists to reference most of the homages, but ESPECIALLY because that last double-splash page is so dense. Also fun for trainspotting!
What I managed to figure out so far with fairly solid degree of confidence (assuming page 1 comes after that recap page):
Pg4 p1 Windsor McKay p2 EC Segar (Popeye)
Pg5 p2 George Herriman (Krazy Kat) p4 Tex Avery/Chuck Jones
Pg6 p5 Will Eisner (Spirit) p6 Jack Cole (Plastic Man)
Pg7 p1 Charles Schulz (Peanuts) p2 Maurice Sendak (Where The Wild Things Are) p3 Dr. Seuss
Pg15 p2 Barry Windsor Smith (Conan) -- Is Mr. Excitement done inna Neal Adams stylee behind Frank Einstein?
Pg16 p5 Geof Darrow
Pg17 p3 Los Bros p5 Daniel Clowes
Pg18 p2 Chris Ware p6 Dave Mazzucchelli (Batman: YO)
Pg19 p5 Brian Bolland p6 Dave Gibbons (Watchmen)
Pg20 p2 Mike Mignola (Hellboy) p3 Bruce Timm (DCAU) p4 Charles Vess p5 Paul Pope (B:Y100) p7 Craig Thompson (Blankets)
Pg21 p2 Frank Cho (Liberty Meadows) p3 Adam Hughes (Mary Jane statue) p4 Frank Quitely p5 Eric Powell (Goon) p6 James Jean
Pg22 (First of double-splash, treating each page individually) p4 Brandon McCarthy (Ronan Gosh) very unsure p6 Jeff Smith (Bone) p17 Frank Espinosa (Rocketo)
Pg23 p1 Jamie Hewlett (Gorillaz) + Matt Groening (my cousin)
At which point I'm stumped, and we're only about halfway through the names on the cheat sheet.

I think the tricky part is that all the homages (or swipes) are in the order they're listed on pg. -2, but not all of those names are actually swiped..
Pg. 4: the Winsor McCay and Segar are followed by... A Barks/Gottfredson panel Hal Foster (Prince Valiant)
Pg. 5: panel 1 is Frank King (Gasoline Alley) panel 2 is Herriman, obviously panel 3 is Milton Caniff (Terry & the Pirates) panel 4 is Tex Avery/Chuck Jones/Looney Tunes Gang panel 5 is Roy Crane (Wash Tubbs)
Pg. 6: top tier is Superman artists: Siegel & Shuster, Wayne Boring, Fleisher Studios middle tier is Batman creators: Bob Kane, Jerry Robinson, Dick Sprang bottom tier is Eisner/The Spirit (left) and Jack Cole/Plastic Man (right)
Pg. 7: In order, Schulz/Peanuts, Maurice Sendak, Dr. Seuss, Hank Ketcham/Dennis the Menace, Charles Addams.
Pg. 8: panel 1: Al Hirschfeld (it'd be funnier if there were a "Nina" in there" panel 2: Bob Montana/Dan DeCarlo (Archie artists) panel 3: Al Capp panel 4: the Harvey artists panel 5: Jesse Marsh/Tarzan
Pg. 9: panel 1: Harvey Kurtzman (MAD-era) panel 2: Will Elder (Goodman Beaver) panel 3: Johnny Craig panel 4: Bernie Krigstein panel 5: Jack Davis
Gotta go to sleep, but I'll try to do some more later, unless somebody else wants to take up the baton.

Time for a few more:
Pg. 10: Would have to be entirely Frazetta, but I don't actually know my Frazetta very well.
Pg. 11: panel 1: Alex Toth panel 2: Wally Wood, the man who said "Never draw what you can copy, never copy what you can trace, never trace what you can cut out and paste down." panel 3: Joe Kubert & sons panel 4: Bruno Premiani (Doom Patrol)
Pp. 12-13: Baffling. The only thing I can think of is Jim Steranko.
Pg. 14: That top Mr. Excitement is definitely Kirby, and there seem to be a few other Kirbys (with various inkers?)
Pg. 15: panel 2, the Madman figure is Barry Windsor-Smith, like you said--and I think the foreground figure in panel 3 is Neal Adams
Pg. 16: panel 1: Hergé panel 2: Moebius panel 3: Yves Chaland panel 4: Daniel Torres panel 5: Geoff Darrow
Pg. 17: panel 1: Dave Stevens (Rocketeer) panel 2: Charles Burns (Big Baby era) panel 3: Jaime Hernandez (I don't really see any Beto in the image) panel 4: Chester Brown (Madman looks like Christian from "Ed the Happy Clown") panel 5: Daniel Clowes
Pg. 18: panel 1: Richard Sala panels 2-5: Chris Ware panel 6: Seth panel 7: Bernie Mireault panel 8: Matt Wagner, I guess (from what?) panel 9: David Mazzucchelli
Pg. 19: panels 1-2, I have no idea panel 3: Kevin Nowlan panel 4: no idea panel 5: Brian Bolland panel 6: Dave Gibbons
Pg. 20: panel 1: Art Adams panel 2: Mike Mignola panel 3: Bruce Timm panel 4: Charles Vess panel 5: Paul Pope panel 6: I see the Jay Stephens but not the Ramona Fradon... panel 7: Craig Thompson
More later, if I get the chance.

I thought Pg. 15, p1 was Steranko? & I guess p4 is Frank Miller? & that drawing of Joe in the upper-right corner of Pg. 14 looks very Romita-esque.

Pg12-13 is the MEANWHILE pages, right? I kind of figured that since we were out of Frank's mind that it was Allred being Allred again.
If Frank Miller is in here, Allred sure hid him well.

I've got the first two issues of The Golden Plates and it's actually pretty good (and very attractive). Haven't tracked down issue 3 anywhere though. Mike said on his own boards that he hasn't go tthe time to do any more issues for the forseeable future.
My Ganrgantua is still with Amazon. Whether they actually have one is questionable.

I nearly went blind trying to figure it all out. The last page (of swipes) and the legend up front have almost nothing to do with each other.
